We would like to share our Advance Care Planning resources during this critical and uncertain time. In anticipation of challenging times ahead due to COVID-19, it is important that we have conversations with our patients in advance of medical emergencies. Research tells us that patients and families want health care providers to initiate these conversations with them and that these discussions lessen anxiety and decrease unwanted and clinically non-beneficial interventions. Advance Care Planning (ACP) and Serious Illness Conversations, which clarify goals of care and decision-making in terms of COVID-19, are especially important for high-risk patients.These materials are intended to support clinicians to start or continue these conversations, and are specific to our situation right now in Fraser Health. Some of the language is broad and exploratory in nature. As resources change or if criteria for access to critical care are established, new materials will be developed to assist you further.
